Second Year Students (September 2011 to April 2012 Intake)

Field Placement Clearance Form Requirements

In order to receive your upcoming field placement assignment, you must bring your First year-Field placement Clearance form, your valid vulnerable sector police record check and CPR Level-C certificate on the Orientation day. All other information regarding your field placement will be given to you on Orientation day, August 31st, 2011.


Field Placement Clearance Form

If you lost or misplaced your First year-Clearance form or you are entering as new second year student, you must download the forms below and have your doctor/physician complete the forms again.

Vulnerable Sector Police Record Check (Please note that the procedure for police check has changed, so please read the instructions carefully)

  • For student who currently lives in Toronto Region (with a postal code that starts with letter “M)

If you need to apply or renew your vulnerable sector police record check and you currently live in Toronto Region, it is mandatory that you contact and book an appointment to see Mohamed Nur at (416) 415-5000 ext. 3689 or  mnur@georgebrown.ca

The authorize representative (s) will give you a copy of the Toronto Police application form for you to complete and sign. Please bring a driver’s license or any type of government photo ID on these days, in order to verify your identification.

Please be advised that Toronto Police Services will take at least 10 to 12 weeks to process your application form. It is very important that you begin the process as soon as possible.

  • For student who resides in Peel, Durham, Halton, York and other region or province

Should you live in another region such as (Peel, Durham, Halton, & York Regional Police website) and other region or province, please check your specific Regional Police Services website (s) in how to obtain their vulnerable sector police application form directly and to find out further information.

Please be advised that the above Regional Police Services will take at least 4 to 6 weeks to process your application form. It is very important that you begin the process as soon as possible.

  • Student with “Positive” Police Record Check Result

A recent RCMP national broadcast has directed all Police Services that fingerprints will be required for positive identification before criminal records are released. Therefore, should any criminal history about you be on file, your application will returned back to you. It will then be necessary for you to have your fingerprints taken as a confirmation of identification.

To do this, please attend the various private RCMP accredited fingerprinting agencies listed in your local telephone book. This process could take an additional 120 days, so start immediately. For more information, please check RCMP website.

Failure to comply with RCMP policy and procedure, will exclude you from being eligible for placement. Once you find out that you have a positive police check result, you must discuss this issue with your Chair or Academic Coordinator.

Standard First Aid with CPR Level-C Certificate (valid every three years)

If you need to renew either your Standard First Aid and or CPR Level-C, you may take these course(s) at any Workplace Safety and Insurance Board (WSIB) approved First Aid Training establishment available in your area.
