2. When does the Health Form need to be completed?
Each program has a specific due date to allow for processing prior to placement. You should start completing your specific program Clinical Pre-Placement Health Form immediately since it may take you 6-8 weeks to complete. You will NOT be allowed to start placement until your health form (including, if applicable, a medical immunizations and lab test results, vulnerable sector police check, mask fit and annual certification of First Aid and CPR courses) has been completed, processed AND approved by ParaMed, the College’s Occupational Health Nurse provider.
3. What could I be asked for?
Depending on your specific program’s health requirements, you may be asked for proof of immunization, laboratory blood test results and non medical requirements such as:
- measles/mumps/rubella (MMR)
- varicella/chicken Pox
- tuberculosis/chest X-ray-annual
- hepatitis B
- tetanus/diphtheria/pertussis-every 10 years
- polio
- flu shot (Influenza)-every Oct & Nov
- vulnerable Sector Police Check-annual
- standard first aid-every 3 years
- CPR level C or HCP courses-annual
- mask fit testing-every 2 years
4. Why do I need a Health Form?
You are required to complete a Clinical Pre-placement Health Form for one or more of the following reasons:
- To meet the requirement for clinical/field placement with vulnerable persons
- To meet a government requirement
- To meet program requirement
- To ensure the safety of the student, patients and clients
5. As a student, what are my responsibilities?
- Make sure that you are registered for the program and paid the tuition fees in full
- You must complete the Important Steps to Follow in your health form in exactly the order listed.
- You must make an appointment with your doctor to complete all the medical immunization and obtain lab test results on the health form.
- If applicable, you must ensure that you have the valid certification for vulnerable police check, Standard First Aid, CPR level courses and mask fit
- Bring the original copy and make a copy of your Clinical Pre-placement Health Form and all the important supporting documents at your scheduled appointment time.
6. Who pays for the health form costs?
The cost of completing the requirements is the responsibility of the student entering the program.
7. How much will this cost me?
Costs will be dependent upon each student’s needs. Doctor’s notes, X-Rays, laboratory blood work report and immunizations may be free of charge or a charge applied by your doctor’s office; this is up to your doctor and NOT the College.
8. Does my program/course cover any of this?
No, however, the mask fitting costs may already be incorporated into your material fees.
9. How long will it take me to complete the Clinical Pre-placement Health Requirements?
If Laboratory (blood work) reports are required, it could take 6-8 weeks for results to be returned to your doctor.

12. If required, where can I have my Mask Fit testing done?
Mask Fit testing can be done at ParaMed during your scheduled appointment. Please do not eat, drink or chew gum at least 20 min before your test, you must be clean shaven.
13. Why do I need to see and submit my health form to the Occupational Health Nurse at ParaMed? ParaMed is the College’s Occupational Health Nurse provider that reviews your confidential medical information and then posts a notification of your clearance status, so your academic coordinator can arrange your clinical or field placement.
14. How do I Create an Account and Book an Appointment on-line with ParaMed?
Once you have confirmed your registration in the program, paid your tuition fee either in full or partially and completed the health form requirements in its entirety, you must go to www.georgebrownhealth.ca and click the Create an Account options and book appointment on-line.
15. What if I’m getting the message "Unable to verify Student ID based on the information provided" during the Create an Account process?

16. Where is ParaMed located?
You must booked an appointment with ParaMed and submit your completed health form at 480 University Avenue, 7th floor, Suite 704 (corner of University and Dundas - St. Patrick subway station). No-walk in visits allowed.
17. What if I have forgotten my password?
You may reset your password in the ParaMed website at www.georgebrownhealth.ca or contact their IT Help Desk between the hours of 9am-5pm, Monday to Friday at 1-866-289-8784.
18. How do I cancel my scheduled appointment with ParaMed?
Students are now able to cancel their own appointment online (with 24 hours’ notice) at www.georgebrownhealth.ca. Failure to do so will result in a fine.
19. What are the additional ParaMed service fees?
As of June 1st, 2011, all new first year students will pay a fee for both the appointment and the mask fit test when you come for your appointment at ParaMed:
- Initial Visit Pricing - $49.40
- Return Visit (due to Deficiency List Form) - $21.80 (fee rates change annually)
- Mask Fit Test - $37.55
- Photocopies - $3.00
*All prices include HST where applicable
20. What happens after my appointment with ParaMed?
ParaMed will enter and update your final clearance health form status online after 48 hours.
21. Where can I view and download my health form information?
Log on to the ParaMed website at www.georgebrownhealth.ca using your
Student ID number and ParaMed password.
22. Can I fax my health form and supporting documents to ParaMed’s OHN?
No, effective January 7th, 2010, ParaMed will no longer accepting fax copies.
23. What happens if my health form is missing information?
- A Deficiency list is created by OHN.
- The list is given to you to complete.
- You will be required to complete the missing information and resubmit documents to the Occupational Health Nurse.
24. What happens when my Pre-placement health form is “Cleared”?
- The health form will be stamped and signed by the Occupational Health Nurse – keep this document and all associated documents.
- Your Academic Coordinators will be notified as to your cleared status.
- Coordinators will notify and assign the student to their clinical/field placement.
