Upon admission and registration
If you are a new first year or current student (Canadian domestic or International) registered in the following programs listed in the left navigation bar beginning in September or January intake, you are mandatory required and expected to complete and submit your program specific Pre-placement health form requirements prior to the school year or health form deadline, in order for you to be qualified and be part of field and clinical placement for the academic year.
Get started, download and information session
You must start the pre-placement health form requirements immediately, as it will take you 10 to 14 weeks or longer to process, complete and submit your health form to ParaMed. To get started, check the left navigation bar and download your specific program code health form. You may also join us at the scheduled information session dates for more information.
Medical immunization and laboratory blood test result
Please read carefully all the specific medical requirements written on your pre-placement health form. It is your responsibility to ensure that your doctor or healthcare professional properly completes, sign and stamp your health form. If you do not have a family doctor, you may visit any Walk-in Clinic available in your area. You should not book an appointment with ParaMed’s Requisite Office until your healthcare professional properly completes all the medical requirements. Otherwise, you will charge a service fee at ParaMed.
Police vulnerable sector check/Standard First Aid/CPR/mask fit test
If applicable to your program, you are also responsible to arrange, pay and complete the additional non-medical requirements at your own time. Please check the left navigation bar for more detail information.
Extra costs, service fees and fine
You will pay extra costs for your ParaMed Requisite office visit, service fees and fine, your doctor’s medical charge for Two Step-TB Skin Tests and/or Varicella and/or Hepatitis B vaccine and for your Standard First Aid and CPR registration, mask fit test and police check application form. All costs, service fees and fine associated with the overall health form requirements are responsibility of the student.
Critical health form deadline
If you fail and do not complete and submit the pre-placement health form and unable to provide a written police vulnerable sector check to ParaMed prior to the health form deadline, your academic program will exclude you from being eligible to attend placement because our agency partners will not allow you to enter and work at their facility. As a result, you will not be able to proceed for next semester.

Once you completed all the pre-placement health form requirements and have confirmed your registration in the program, you must create an account and book an appointment with ParaMed Requisite Office at www.georgebrownhealth.ca and bring the original and photocopies of your health form documents and your fee payment at your scheduled appointment.
