We urge you to start the Clinical Pre-placement Health Form requirements immediately, since it will take you 6-8 weeks to complete. It is your responsibility to ensure that your doctor or health care provider properly complete all the medical requirements, signed and stamped your health form. If you do not have a family doctor, you may visit any Walk-in Clinic available. The cost of all immunizations and non-medical requirements are paid for by the student.
Failure to have the proper health forms and supporting documents completed and submitted to ParaMed prior to the deadline will result for the student being excluded and ineligible for placement and therefore unable to proceed to the next semester.
So to get started, check out the left navigation bar and download your specific program health form. Please read the instructions carefully and use the links and information that will help and guide you to successfully complete all of the requirements.
Once you completed all of the health form requirements and have confirmed your registration in the program, you must create an account and book an appointment with ParaMed, the College’s Occupational Health/Registered Nurse provider on their website at http://www.georgebrownhealth.ca (with the exception of C116, C136, C405 programs). ParaMed is located at 480 University Avenue on the 7th floor, Suite 704 (Northwest corner of University and Dundas, beside CIBC Bank), (St. Patrick Subway Station). No walk-in visits allowed.
Please bring the original and photocopy of your health form and other important supporting documents at your scheduled appointment. After your appointment with ParaMed, please allow three business days to update your personalized and confidential health form account and final clearance status online.
Should you need to cancel your appointment with ParaMed (with 24 hours notice), just sign-in with your Student ID# and password and follow their instructions carefully on their website at http://www.georgebrownhealth.ca. If you fail to provide 24 hours notice, the College will send you an invoice to pay a fine. (Fee rates vary annually)
