Disability Services

PROCEDURES TO REGISTER FOR DISABILITY SERVICES

To access Accommodations at GBC, you must register with Disability/Deaf and Hard of Hearing Services.

To Register:

Complete the Disability/Deaf and Hard of Hearing Services Intake form and attach copies of documentation which clearly specifies the nature of your disability and accommodations required. Missing information may delay the processing of your application and access to services.

  • If medical: Speak to your doctor and ask him/her to write a short letter explaining the nature of your disability, your medications, and how frequently you see the doctor for treatment. If your doctor has any specific recommendations regarding your participation in a college program, please have him/her specify the conditions and procedures you require to ensure your safety or you can have your doctor complete the Standard Medical Form.

  • If Psychological (including Learning Disability): Contact your Psychologist, school or school board to provide you with the appropriate documentation or you can have your doctor complete the Standard Medical Form.

  • If Visual: Please enclose a copy of your CNIB Registration card.

  • If Deaf or Hard of Hearing: A report from your Audiologist or proof of attendance at a Provincial School for the Deaf is required.

FAX a copy of your Intake Form and documentation to (416) 415-2726.
